1.You should have a word with the primary doctor and find if they agree to sign a medical marijuana doctor’s statement. Make sure that the doctor signing this form is an MD or DO. And if the doctor isn’t ready to sign, you would then have to get your medical records stating your condition. Fix an appointment at a medical marijuana clinic, and don’t forget to carry the medical records you obtained from your licensed medical marijuana doctor. The statement then has to be filled out and signed from the clinic.
2.The next step is to fill the OMMP application with yours, your caregivers, and grower’s information. If there isn’t a grower or caregiver, you may leave the section blank.
3.Once the paperwork has been taken care of including OMMP Application & doctor’s Statement, get yourself copies of the application and statement.
4.Now you have to mail your original documents to the OMMP through certified mail receipt for the USPS. Don’t you forget to ask for the a return receipt so as to know that the paperwork has been received by OMMP.
5.Ensure that you carry all of your paperwork all the time the way you would carry your OMMP card.
6.You would then have to wait for the medical marijuana (MMJ) card that allows you to buy medication from an Oregon Medical Marijuana dispensary.
